December 17, 2020
To the valued patients of our Jackson Family Dentistry community,
In January 2019, I purchased Jackson Family Dentistry from former owner, Dr. Scott Charmoli, who continued to see patients through August 2019. After Dr. Charmoli’s departure from Jackson Family Dentistry, while reviewing files of past patients, I discovered what I believed to be evidence of potential misconduct / unethical practices during his tenure.
The health and safety of patients is my highest concern as a doctor. As medical professionals, we take an oath to “do no harm” to our patients, which is why I felt the ethical obligation to report activity that I believed to be suspicious. On December 15, Dr. Scott Charmoli was indicted by the US Department of Justice on ten counts:
At Jackson Family Dentistry, our goal is to provide quality care that improves your health, comfort and appearance. We understand that the opportunity to care for you is a privilege, and one we take very seriously. My heart goes out to any patients who may have been mistreated in this situation, and my goal is to help rebuild their trust and faith in the dentistry profession through excellent care and patient service.
While this is a deeply disturbing matter with serious implications for patients who received care under the practice of former owner Dr. Scott Charmoli, it is a separate and distinct matter that has no impact on the current or future level of excellent care that patients receive at Jackson Family Dentistry under its current ownership.
Without exception, dental care should be built on a foundation of honesty and earned trust. I welcome the opportunity to sit down with you to personally review your records and address any concerns you may have about your dental health.
Thank you,
Dr. Pako Major

On December 15, Dr. Scott Charmoli was indicted by the US Department of Justice on ten counts:
The health care fraud charges are in regard to the money Dr. Charmoli collected for the alleged unlawful procedures, both from insurance companies and patients directly.
The false statement charges assert that Dr. Charmoli knowingly and willfully made a false statement to a patient that his/her tooth needed a crown when, in fact, Charmoli damaged the tooth intentionally in order to deliver the service and collect payment for the service of administering a crown.

Dr. Major suspected misbehavior after randomly reviewing patient records, going back at least five years. Upon caring for patients under new ownership, evidence of potential misconduct / unethical practices by the former owner were discovered and reported to the Wisconsin Department of Safety and Professional Services and the U.S. Department of Justice.
At this time it is difficult to say how many patients were affected by the misconduct under investigation. If you believe that you were affected by Dr. Charmoli’s misconduct, please contact Jackson Family Dentistry.
All former patients of Dr. Charmoli are in the process of being contacted. If you believe you have been affected by the misconduct of Dr. Charmoli, please contact Jackson Family Dentistry.
The dental practice was purchased in January 2019 with zero knowledge of or association with the alleged actions of Dr. Charmoli that are currently under investigation. While some of the staff have stayed at the practice through the transition of ownership, they do not have the expertise nor the access to have been aware of the alleged misconduct.
